Proxy Access and Third Party Consent
At our surgery, we understand that sometimes patients may need support managing their healthcare. This can include allowing a trusted family member, carer, or representative to access medical information or manage appointments and prescriptions on their behalf.
This page explains how proxy access and third party consent work, who can apply and what steps need to be taken to ensure access is safe, appropriate, and in line with NHS confidentiality and data protection guidelines.
Please read the information below carefully if you wish to give someone access to your medical record, or if you are requesting access on behalf of someone else.
Proxy Access
Proxy access is when a trusted person, like a family member or career, is granted online access to a patient's GP services such as making appointments or ordering prescriptions.
This is common for parents managing a child's health or careers assisting an adult who cannot manage their own affairs.
Third Party Consent
Third party consent allows a patient to grant a trusted individual permission to access their medical information or manage their services on their behalf.
How to Gain Proxy Access or Third-Party Consent
To gain proxy access or third-party consent, please complete one of the following forms:

Consent to Share Health Information with Named Individual (Third-Party Consent)
This form authorises someone to access information about you and your medical conditions, speak to us about your care, and both give and receive information on your behalf.
As the patient, you will need to complete and sign the Consent to Share Health Information with Named Individual form and return it to reception along with a valid form of identification (ID).
If you are unable to provide consent, a legally authorised representative (e.g. someone with power of attorney) may complete and sign the form on your behalf. They must provide valid ID and legal proof of their authority.
Our Care Navigation Team will verify the ID and ensure the form has been completed in full. Once verified, your medical record will be updated accordingly.
Application for Proxy Access – Under 12 Form
This form allows a parent or guardian to access a child’s online medical record, view test results, request repeat prescriptions, and book appointments.
As the parent or guardian, you must complete the Application for Proxy Access – Under 12 Form and return it to reception with valid ID for both yourself and the child.
Our Care Navigation Team will verify the ID and ensure the form is fully completed. Once verified, the child’s medical record will be updated to reflect the proxy access.
Application for Proxy Access – Over 12 Form
This form allows a nominated individual to access a young person's online medical record, view test results, request repeat prescriptions, and book appointments.
The young person must complete and sign the Application for Proxy Access – Over 12 Form and return it to the surgery in person, along with a valid form of ID.
Our Care Navigation Team will confirm with the young person that they have completed the form willingly. They will also inform them that their GP may contact them to verify the information provided. The team will then verify the ID and ensure the form has been completed in full.
The completed form will be passed to the patient’s usual GP, who will review the form and medical record to ensure there are no concerns with granting proxy access.
If no concerns are identified, proxy access will be granted, and login details will be provided to the nominated individual.
Application for Proxy Access – Adults Form
This form allows a nominated individual to access an adult patient’s online medical record, view test results, request repeat prescriptions, and book appointments.
The patient must complete and sign the Application for Proxy Access – Adults Form and return it to the surgery in person, along with valid ID.
Our Care Navigation Team will confirm that the patient has willingly completed the form and inform them that their GP may contact them to verify the details. The team will then verify the ID and ensure the form has been completed in full.
If proxy access is being requested due to a lack of capacity, this will be flagged for GP review. Supporting documentation, such as a legal power of attorney, must be provided.
The completed form will be reviewed by the patient’s usual GP to determine whether there are any concerns that would prevent proxy access from being granted.
If no concerns are identified, proxy access will be granted, and login details will be provided to the nominated individual.
