How to give patient feedback

You can give patient feedback by completing our online form.

You can also give feedback by contacting the surgery directly or by completing one of our comment cards at the surgery.

How to make a complaint about an NHS service

If you're unhappy with an NHS service, then it's worthwhile talking about your concerns with us early on, as we may be able to sort it quickly.

If you would prefer to speak to someone else outside of our surgery, then you can contact the Patient Advice and Liaison Service (PALS). This is a free, confidential and independent service that you can use to talk to someone who understands the complaints process and can offer some guidance and support.

As a family we have all been poorly for a month, we’ve had the most fantastic service from CD surgery. My youngest’s chest infection hadn’t gone away, from submitting an online request to a response from a doctor was 19 minutes; incredible service. Thank you to all the staff, friendly and professional from first contact to seeing a nurse and a doctor.

I just wanted to let you know that I think it’s absolutely fantastic. The experience is far better than having to wait to call at 8am and hope you will be fast enough to get an appointment. Each time I have used it my form has been triaged within 2 hours and an outcome of either an appointment that day (for my young son) or in a few days time for myself. I think it is a really positive change.
